Prevent patient change after check-in

Description

Appointments can be edited to change both the customer and patient after Check-In.

If the patient is removed, this will lead to “Context doesn't specify a patient” errors at Consult.

To avoid this, make the Customer and Patient read-only once an appointment is checked-in
