Cancel reminders where the reminder type has been deactivated

Note that this will:
cancel In Progress reminders when the Patient Reminder Queue job runs
cancel queued reminders items, when the Patient Reminder Sender job runs The associated reminders won't be cancelled by this job, however these will (eventually) be cancelled by Patient Reminder Queue job. This is in keeping with the current behaviour for handling deceased and inactive patients, and inactive customers.
