Can't create patient alerts in visit editor

Description

As part of the changes for OVPMS-1885, the Reminders/Alerts tab in Patients - Medical Records was split into separate Reminders and Alerts tabs.

In the Visit Editor, the Reminders/Alerts tab was renamed Reminders, and allows reminders and alerts to be displayed. However, while alerts can be edited, they no longer be created, and the description is no longer displayed.

Split the alerts functionality into a separate tab, the same as Patients - Medical Records.
