The changes for included support add clinical notes associated with top level templates to the patients history when invoicing estimates in the Visit editor.
This needs to be expanded to:

• include clinical notes from top-level templates when invoicing estimates in Customers - Estimates

• include clinical notes from top-level templates when invoicing in Customers - Charges

Estimates and invoices in the Customers workspaces may have multiple patients, so clinical notes need to:

• be added to each affected patient

• create Visits for a patient if they do not have one
