Distinguish between SMS appointment reminders and general SMSes in communications log

Description

Currently appointment reminder SMSes and those generated manually are logged in the communication log with the subject and reason 'Ad-hoc SMS'.
Automatic appointment reminders should be instead be logged with the subject 'SMS Appointment Reminder' and reason 'Appointment Reminder' to distinguish between how they were generated.
