Order items subreport does not allow for large total order amount

If the total amount of an order gets large, the total is not displayed. Solution is to make the total fields wider. The attached is the fixed A4 version. I also fixed a couple of other problems: a) for tax-ex countries (US, Canada) the total ex-tax amount had label 'Invoice Total' - should be 'Order Total'; b) in no-tax countries (eg Hong Kong) the 'Includes Tax' line was displayed - should be suppressed.
