Refreshing a patient letter with no document displays incorrect message

Description

If a patient letter is created by invoicing a product, it doesn't have any associated document.
This is automatically generated when the patient letter is printed, or refreshed.

When clicking Refresh for such a letter, a dialog displayed with the message:

1 2 3 Refresh the contents of null? Any changes will be removed.

This is because it is expecting the file name of the document to be populated.
Where there is no file name, it should instead display the template name.
